As a registered provider or person in charge of a special care unit, you are legally required to notify us within certain time frames about certain incidents, events or changes within your centre.

You are required to notify us of any changes to information supplied for the purpose of registration.

To assist you to meet this regulatory requirement, we have designed a suite of notification forms.

Person in charge

  • Any injury to a resident other than those previously notified in NF03
  • Any loss of power, heating or water
  • Any occasion of fire alarm activity
  • Any occasion where physical, environmental or other restraint was used

Submission dates for return of the quarterly notifications are:

  • 31 January (for incidents that took place in October, November and December)
  • 30 April (for incidents that took place in January, February and March)
  • 31 July  (for incidents that took place in April, May and June)
  • 31 October  (for incidents that took place in July, August and September)

You will need to download the notification forms below. It is not possible to submit these notification forms using the portal.
